Health Insurance Services
Community Health Initiative offers health insurance enrollment and re-enrollment assistance in Covered California, Private Individual and Family Plans, Medi-Cal, Kaiser Child Health Program and Medicare! We can also help you obtain dental insurance coverage.
Case Management Services
Once our Health Access Specialists have assisted you in enrolling in the appropriate plan, they will follow up with you to make sure your application is approved, that you receive your insurance plan cards, and that you are able to access services. We will contact you twice a year to survey your satisfaction and usage of the insurance plans. When the time comes for you to renew your eligibility, they will assist you in completing the appropriate forms and submitting all required documentation.
Information Needed to Apply
- Proof that you live in Napa County, such as a utility bill or rental agreement with your current name and address listed.
Proof of income, with one of the following:
- Recent paycheck stub; previous year’s federal income tax return; written statement from your employer stating gross monthly income and when it was earned; OR
- Persons paid in cash may complete and sign a form stating income and when it was earned; OR
- Self-employed may use the previous year’s federal income tax return with Schedule C.
- Birth certificate
Health Insurance Plans
Medi-Cal is a State run program and it is California’s version of Medicaid. This is a public health insurance program which provides needed health care services for low-income individuals including families with children, adults, seniors, persons with disabilities, foster care, pregnant women, and low income people with specific diseases such as tuberculosis, breast cancer or HIV/AIDS. An individual has access to full services for medical, dental, vision and mental health.
Covered California is the health insurance marketplace in the U.S. state of California. Because of the Patient Protection and Affordable Care Act (ACA), the exchange enables individuals and small businesses to purchase health insurance at federally subsidized rates. Individuals that are eligible are required to have health insurance as part of this federal law. Also recognized as “Obamacare”. This marketplace enables individuals to shop for health insurance and pick a plan that best suits their needs.
Private Insurance is purchased directly from the Health Insurance Company. There are no subsidies provided so any person can apply for this type of coverage.
Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD).
Health insurance eligibility
Eligibility for Medi-Cal and Covered California is based on many different factors including:
- Annual Income
- Number of people in your household
- Age of people in your household
- Number of people you claim on your taxes
- If anyone in the household is pregnant, disabled or blind.
